You fax, scan and email all of your paper medical records to LOK and we’ll provide the Upload Service to attach your records to your PHR (up to 300 pages per 2 year membership). Includes our Premier Plan services described above. In addition, we’ll categorize all entries into your PHR by date and service. You provide authorization and we contact up to 3 of the following providers: doctors, labs, pharmacies and hospitals.
Once we have your health information we digitize the information with the goal of doing so in 30 days or as soon as your provider answers the request for information.
Additional Provider Fees. Sometimes physicians and other medical providers charged a nominal fee to provide a copy of your records. Any provider fees are included in the price.

| Billing Information |
Your membership includes a one-time non-refundable registration fee of $4.95. The remainder of your membership will be billed at a pro-rated monthly rate based on your annual membership. For example if the membership is $64.95 then $4.95 is the non-refundable registration fee and the remaining $60 is billed at $5 per month.
Please note:
- You will receive a monthly charge on your credit card statement for PHR membership.
- Portable PHR devices will be charged one time the full amount on your credit card statement.

- PHR Memberships are not taxable.
- Portable PHR devices are charged shipping and handling (in compliance with New York State laws all New York State residents will also be charged sales tax.)

| Cancellation Policy |
| Within the first 30 days of purchase, you can cancel your membership for a full-refund (except for the $4.95 registration fee). After the first 30 days, your refund will be adjusted based on the number of unused months. No refund will be provided for the month you terminate your membership. |

| Cancellation Policies for Pregnancy and Family Memberships |
| In addition to our standard cancellation policy above, Pregnancy and Family memberships are subject to the following: |

| Pregnancy Memberships: |
Pregnancy is a 12 month membership; plus you receive a PHR free for the first six months for your newborn.
- If the mother’s membership is canceled at any time during the 12 months the 6 months free for newborn offer is forfeited.

| Family Memberships: |
- The pricing for family memberships is based on each incremental family member receiving an incremental discount. So, if one member of a family chooses to cancel the PHR membership—this effects the pricing of the entire family membership. The more family members you have signed up under one plan, the greater the incremental discount you will receive.
- When a user cancels his membership as part of the family membership plan, the user must identify specifically which family members or all family members being canceled. Any family members not specifically identified as canceled will continue to be charged each month.

| Exchange Policy |
To accommodate the changes in your life stage and health and to insure you’re using the LifeOnKey PHR product to best suits your needs, we provide a flexible policy to enable you to move from one PHR product to another.
If you decide to move from one PHR to another, such as switching from Total Health to Pregnancy, which has different annual membership price, you will simply pay the cost for the new product. For example, if you subscribe to Total Health and after 3 months you learn that you are pregnant and need to use our Pregnancy PHR, you will be billed monthly at the Pregnancy PHR rate.
When you change plans you will not have to pay a new registration fee. If you wish to move to a lower-priced PHR, say you subscribe to Diabetes and want to move to Total Health after three months, you would simply pay for Total Health, bringing your monthly fee down.
