| LifeOnKey™ — an open platform system that shifts the way information is managed. |
| LifeOnKey™ opens the door to clients and third parties to customize solutions, empowered to embrace and extend current technology on a platform which transfers the management of healthcare technology into the modern age. |
| LifeOnKey is an interoperable, open system that provides three integrated applications aimed at the major users of health data: |
| Online health information management solution that: |
Enables communication of medical data and information across the health landscape |
Designed to aggregate, disseminate and share medical data between physicians, consumers, patients, hospitals, labs, pharmacies, payers, pharmaceutical companies, marketers and researchers |
Applications meet all meaningful use standards and serve as a gateway for health care providers and institutions to share, transform and analyze data |
Enables consumers/patients to change how they manage personal health |
Easy integration with legacy systems: can serve as a gateway and provide a platform to expand, extend and transform the collection and sharing of patient data |
| Unlike stand-alone products offered by many health information technology companies, LifeOnKey’s three applications are seamlessly integrated. This is our distinct advantage: we deliver relevant data that can be refined and targeted to the needs of each unique customer. |

| ecosystem |
| Here’s how it works: |
LifeOnKey builds a web service – essentially a secure pipeline – between your database and ours. |
Every time a consumer logs into his or her PHR, the service connects to your database and those of other external providers, checks for new data on that consumer and automatically feeds that data into the LifeOnKey system, where it is integrated and displayed to the consumer. |
LifeOnKey values the data, and co-shares revenues with the Ecosystem partners. |

| health plans |
| Enhancing the health of plan members can be a huge lever for insurance companies looking to lower the total cost of health care delivery – and PHRs are an important instrument in that regard. |
| Through LifeOnKey’s PHR, individuals can create a complete health picture, bringing together a full personal medical history as well as information and digital images from all health care providers. No other health record offers such a robust, easy-to-use combination of customized tools, convenience and control – accessible in multiple languages from multiple technology sources. |
| By making LifeOnKey PHRs part of its standard benefits package, an insurer can help healthy members stay healthy, while making it easier for members with chronic conditions (e.g., diabetes) to manage their day-to-day monitoring and care. |
Enabling individuals to take charge of their health and wellness – PHRs provide an easy, convenient, secure vehicle for individuals to become more involved in monitoring their own well-being. As a result, PHR users are likely to be more focused on prevention, scheduling timely checkups and tests, following a sensible diet and getting regular exercise – behaviors that yield lower health care costs over the long run. |
Improving care of chronic illnesses – LifeOnKey provides easy-to-use tools to help patients cope with different diseases and stages of life, enabling better compliance with doctors’ orders and ultimately lower health care costs. For example, a diabetic can instantly upload glucose monitoring results, record food intake and exercise levels, and communicate with his or her doctor about adjusting insulin levels as needed. |
| Provides Access to Valuable Data |
| LifeOnKey’s platform was specifically designed to aggregate completely anonymous, non-identifiable information for the research community while ensuring the highest levels of security, privacy and authentication. Our attention to data architecture makes LifeOnKey a powerful platform for health care providers and payers to analyze performance and improve medical processes and delivery of care. |
On-demand access to real-time, anonymous, longitudinal data – Insurers can collect, store and analyze data for their own members and compare anonymous patient data from other institutions regionally, nationally and internationally. In other words, insurers gain access to a wealth of data beyond their own customer base, allowing them to benchmark performance and outcomes across multiple dimensions. |
Sophisticated data-mining capabilities – LifeOnKey’s Research Center application allows users to develop a comprehensive analytical view of targeted populations based on disease, demographic criteria or other factors. As a result, insurers are better equipped to monitor compliance among particular segments, spot emerging trends, influence policy and practice, and cut costs in long run. |
| Aids Retention of Corporate and Individual Customers |
Nearly all companies are looking for ways to rein in the cost of providing health care coverage for their employees. Offering timely data and trend analysis that helps employers get a better handle on health care costs can enhance the insurance company’s value proposition. |
Likewise, for individuals who shoulder the full cost of their coverage, an insurer-provider PHR that includes free tools and information may be a valuable retention tool. |
| Offers Low Total Cost of Ownership |
| Most health IT products require heavy up-front costs, replacement of existing systems and annual license, maintenance and upgrade fees – as well as costly consulting engagements to implement and integrate these systems. LifeOnKey is different – a cloud computing solution that requires minimum implementation and support resources. |
Compatible With Legacy Systems – LifeOnKey works with any other application and database, allowing it to run side-by-side or above legacy systems and making it easy to install, manage and maintain. |
Easily Tailored – Our flexible platform enables insurers to offer their members a customized, co-branded solution powered by LifeOnKey. |
| Encourages Adoption of EHRs by Physicians |
Three out of four unaffiliated doctors in the US do not have a health information management solution. Starting in 2011, U.S. government incentives for modernization of medical record keeping and “meaningful use” of health IT may encourage these doctors to install EHRs. |
LifeOnKey can serve as the middleware platform for integrating physicians’ data on insurance company members. This would require members to authorize such information sharing or physicians to transfer anonymous data. |
| Provides Consistently Comprehensive View of Members |
| By equipping new and existing members with a PHR, insurance companies gain a clearer, more complete profile of their customers, enabling more efficient operations. |

Historically, clinical trial data were collected at the clinical research site on pieces of paper designated as Source Documents. |
Electronic Data Capture (EDC) is the current manifestation of the past process. |
EDC allows users to enter, review, and analyze data in real time. The EHR allows users at a clinic or hospital to do the same for patient care data. The convergence of EDC and the EHR is inevitable and has the potential to improve the management of clinical research data and physician management of their clinical research operations. |
While the EDC and EHRs are similar in both structure and function, as both focus on collecting patient data, adoption of both systems is still relatively modest. When properly designed, integrated EHR and EDC solutions should offer a convenient, cost-effective one point of data entry for the clinical sites to enter data. As a result, in the future, both traditional paper and paper source documents could be eliminated. |
The pharmaceutical industry will no longer focus on how well people can transcribe data from one medium to another; instead, companies will focus, in real time, on how well the sites are following the protocol and assuring a high level of data quality and patient safety. |
LifeOnKey can provide CROs with a middleware solution to be the integration platform between EDC and EHR for their clinical trials, as well as PHR for the clinical trials participants, and a data center for the de-identified collective data for the Pharma. |
LifeOnKey is an excellent platform to then help the Pharma company in their Phase 4 studies. |

| Provides Comprehensive, Long-Term View of Patient |
Unlike other Patient Health Records, LifeOnKey’s patient data is not captive to a single health care institution – whether insurer or health provider. This provides pharmaceutical companies with a comprehensive, long-term view of the patient, not simply a snapshot from a single health information source nor an extract from an electronic medical record. |
With this advantage, pharmaceutical firms can track specific patient groups, following product use and patient outcomes over the long term. By drawing upon LifeOnKey’s de-identified, aggregated patient data, pharmaceutical companies can isolate health trends and product usage, and analyze long-term outcomes important to new drug development, drug safety and overall research. |
| Enhances Data Integrity and Data Portability |
LifeOnKey is built to aggregate anonymous, de-identified data. The system’s architecture and database procedures were designed to completely safeguard all personal medical information, giving patients the confidence they need to create full health histories based on complete medical information. |
Since individual patients control their data and because their data is fully portable – not restricted to a single insurer or provider – researchers are more likely to have access to data that only patients know, such as family health history, lists of non-prescription medications being taken and other information that might not be captured by an insurer-provided PHR. |
| Accelerates and Optimizes Recruitment for Clinical Trials |
Through its Research Center, LifeOnKey offers a dedicated application for research and clinical trials that provides on-demand access to real-time, anonymous, longitudinal data. Anonymous data can be compared across health care institutions regionally, nationally and internationally. |
LifeOnKey provides a unique Profiling Module that helps pharmaceutical companies identify relevant patient profiles and anonymously approach patients for participation in clinical trials. |
LifeOnKey patient profiles are auto-populated with data from all external sources. Data mining can be mapped to specific needs based on disease, demographic sector or customer base, providing a comprehensive analytical view of the targeted populations and accelerating recruitment for clinical trials. |
Unlike traditional point solutions, LifeOnKey offers the industry’s only end-to-end Clinical Trial Module which integrates PHR, EHR and clinical trial data. This module fully supports Phase 4 testing. |
| Facilitates Communication and Educational Opportunities Between Pharmaceutical Companies and Patients/Consumers |
LifeOnKey creates multiple opportunities for pharmaceutical companies to educate and communicate with patients. Closed communities allow pharmaceutical companies to communicate and interact directly with patients. |
Through LifeOnKey, companies can reach patients/consumers with advertising and information targeted to medication, condition, diagnosis or other data points in a PHR. Since medical data is de-identified, direct communications with patients can be conducted anonymously, privately and confidentially, but with the assurance that each patient meets the criteria targeted by the company. |
LifeOnKey creates an important new channel that allows pharmaceutical companies to rapidly convey information to targeted audiences regarding drug safety, adverse reactions or other health issues. |
| Encourages Adoption of EHRs by Physicians |
Three out of four unaffiliated doctors do not have a health information management solution. |
LifeOnKey’s easy-to-implement, affordable EHR module can accelerate adoption by physicians and provide an information link between pharmaceutical companies and physicians. |

Technology & Security
LifeOnKey™ is a revolutionary Personal Health Record (PHR) platform that allows you to control your health information in a completely secure, safe and integrated way. At the core of the LifeOnKey™ PHR platform is a proprietary, market-proven Secure-Safe Privacy-Plus (SSPP) technology that delivers a state-of-the-art interoperable system.
Your Security and Privacy is Assured
LOK’s Secure-Safe Privacy-Plus (SSPP) patent-pending technology ensures that your health information is secure, using multiple security socket layers to house and separate your personal and medical data in different places.
The SSPP system goes beyond standard security measures and requires you to enter four pieces of information before you can access your information:
- Username
- Password
- Country of Residence
- Unique User ID
Only when you use your password and encrypted ID will your health information—from your various health providers, hospital, lab or pharmacy—be connected and gathered in one place. Only when you decide to compile your LifeOnKey file does the data merge into your own personal computer’s browser. With the LOK technology the data never stay on your computer cache.
The LOK technology complies with HIPAA, the federal standard for the protection of personal health records to ensure the privacy of electronic transmission of health information.
What does interoperability mean and why is it important?
It means: The LifeOnKey™ system is smart.
The LifeOnKey™ "interoperability module" automatically and seamlessly collects all your medical data independent of source and format—and integrates and displays the information through a secure information network.
That means LifeOnKey™ has the ability to collect selected information from the various databases of insurance companies, hospitals, doctors’ offices and other health care providers. LifeOnKey™ lets you control and decide what information you want to store—and how. Not only can you build a virtual file for yourself when you need it, you can also save a local file where you need it—your personal computer, PDA or memory stick.
That’s really important for medical centers, physicians, administrators, and for you. Why? Because you are no longer burdened by having to collect the various information from the medical organizations that keep your health information. LifeOnKey™ will configure and fit to all systems.
With more than one million LifeOnKey™ members currently using our PHR platform and scores of different information systems being utilized by those members, the most important part of LifeOnKey’s smart-platform is this: it works.
Market-Proven Innovation
The sophisticated technology of LifeOnKey™ “sits” on top of existing health information management systems and data uploaded from peripheral medical devices.
The system provides easy-to-use patient rights-control modules, management tools, tracking of follow-up procedures, and prevention strategies. Most importantly, LifeOnKey™ ensures the privacy of your personal health information.
The business intelligence embedded in LifeOnKey™ will also allow you to view your health status privately and securely, against a larger LifeOnKey population. Imagine being able to compare your treatment or outcomes against a real-time, documented view of people with similar ailments or diagnoses—all real health data aggregated, never individual information.
The Intersection of Technical Security and Consumer Confidence
LifeOnKey's secure network is managed on a continuous on-demand basis, empowering you to track and manage your medical condition or health goals. This tracking is based on your auto-populated personal health record and any health information you have uploaded to the system from external medical devices. This allows for integrated management of special health conditions, and relevant health markers such as stress cortisol, blood pressure, cholesterol count, and blood sugar levels. LifeOnKey™ members are able to upload confidential and potentially lifesaving information and also track lifestyle behaviors—like diet and exercise—to promote self-care and preventive health.
Meeting the Needs of Consumers
LifeOnKey™ was specifically designed to answer consumer and patient needs, which have been recognized by leading health care leaders such as the Agency for Healthcare Research & Quality, The Robert Wood Johnson Foundation and The Markle Foundation:
- Individuals should be able to access their health and medical data conveniently and affordably
- Individuals should be able to authorize when and with whom their health data are shared
- Individuals should be able to designate someone else, such as a loved one, to have access to and exercise control over how their records are shared
- Individuals should receive easily understood information about the ways their healthcare data may be shared
- Individuals should be able to review which entities have had access to their personal health data
- Electronic health data exchanges must protect the integrity, security, privacy, and confidentiality of an individual’s information.
LifeOnKey's demonstrated commitment to giving consumers and providers the ability to electronically share health information securely while protecting patient privacy is also aligned with the national strategy set by the Office of the National Coordinator for Health Information Technology.
LifeOnKey, Inc. is committed to providing the most reliable, innovative technology to its worldwide network of members and partners. With over 20 Centers of Excellence worldwide, LifeOnKey provides leading-edge technological advances combined with ‘real-world’ experience—all in an unrelenting effort to provide you with a secure, consumer-centric personal health record platform.

Target Health, Inc. and LifeOnKey, Inc. Team Up To Bring State-Of-The-Art Information Technology System to Clinical Trial Process |
| INNOVATIVE PARTNERSHIP HELPS IMPROVE MEDICAL RESEARCH BY OPTIMIZING CLINICAL TRIALS |
| Target Health, Inc. and LifeOnKey, Inc. Team Up To Bring State-Of-The-Art Information Technology System to Clinical Trial Process |
| (July 20, 2010 – New York, New York) Target Health Inc. and LifeOnKey, Inc. today announced they have joined forces to offer an innovative, web-based information platform that will allow the bio-pharmaceutical and medical device industries to optimize clinical trials, help researchers capture, analyze and assess trial data more efficiently, and bring innovative new medicines and devices to patients. |
| Through its partnership with LifeOnKey, Target Health can provide health care companies with an electronic clinical trial record that can manage and monitor the care of clinical trial patients from Phase I through Phase IV post-marketing surveillance. |
| By combining Target Health’s clinical trial software with LifeOnKey’s electronic health records (EHRs), the partnership will give clinicians online access to the most recent medical information for their clinical trial patients, allowing them to efficiently track critical information – from side effects to health improvements, while maintaining the integrity of the clinical trial data. Meanwhile, patients will gain the ability to follow the progress of the clinical trial through their own patient health record. |
| “We are very enthusiastic about building a comprehensive solution that integrates clinical trial software and EHRs – the first offering of its kind for this industry,” said Target Health President Dr. Jules Mitchel. “This partnership leverages the deep expertise with clinical trials and information systems within both companies to enhance the efficiency of the drug, biologic and device development process.” |
| “This partnership clearly highlights the ultimate promise of health information technology,” said Dr. Linda Harnevo, CEO of LifeOnKey. “Through the smart application of information technology, we can help clinicians improve the clinical trial process from end-to-end – from better patient recruitment to more effective patient monitoring once the drug is on the market.” |
| According to Dr. Mitchel and Dr. Harnevo, the Target Health-LifeOnKey partnership will also ensure better compliance with clinical trial requirements and help medical researchers track specific patient groups, follow product use and evaluate patient outcomes over the long term. |
| About Target Health, Inc. |

| 2. |
Portable PHR devices will be charged one time the full amount on your credit card statement. |

| Cancellation Policy |
| Within the first 30 days of purchase, you can cancel your membership for a full-refund (except for the $4.95 registration fee). After the first 30 days, your refund will be adjusted based on the number of unused months. No refund will be provided for the month you terminate your membership. |
| Cancellation Policies for Pregnancy and Family Memberships |
| In addition to our standard cancellation policy above, Pregnancy and Family memberships are subject to the following: |
| Pregnancy Memberships: |
| Pregnancy is a 12 month membership; plus you receive a PHR free for the first six months for your newborn. |
If the mother’s membership is canceled at any time during the 12 months the 6 months free for newborn offer is forfeited. |
| Family Memberships: |
The pricing for family memberships is based on each incremental family member receiving an incremental discount. So, if one member of a family chooses to cancel the PHR membership—this effects the pricing of the entire family membership. The more family members you have signed up under one plan, the greater the incremental discount you will receive. |
When a user cancels his membership as part of the family membership plan, the user must identify specifically which family members or all family members being canceled. Any family members not specifically identified as canceled will continue to be charged each month. |
| Exchange Policy |
| To accommodate the changes in your life stage and health and to insure you’re using the LifeOnKey PHR product to best suits your needs, we provide a flexible policy to enable you to move from one PHR product to another. |
| If you decide to move from one PHR to another, such as switching from Total Health to Pregnancy, which has different annual membership price, you will simply pay the cost for the new product. For example, if you subscribe to Total Health and after 3 months you learn that you are pregnant and need to use our Pregnancy PHR, you will be billed monthly at the Pregnancy PHR rate. |
| When you change plans you will not have to pay a new registration fee. If you wish to move to a lower-priced PHR, say you subscribe to Diabetes and want to move to Total Health after three months, you would simply pay for Total Health, bringing your monthly fee down. |

| LifeOnKey - Technology & Security |
| LifeOnKey™ is a revolutionary Personal Health Record (PHR) platform that allows you to control your health information in a completely secure, safe and integrated way. At the core of the LifeOnKey™ PHR platform is a proprietary, market-proven Secure-Safe Privacy-Plus (SSPP) technology that delivers a state-of-the-art interoperable system. |
| Your Security and Privacy is Assured |
| LOK’s Secure-Safe Privacy-Plus (SSPP) patent-pending technology ensures that your health information is secure, using multiple security socket layers to house and separate your personal and medical data in different places. |
| The SSPP system goes beyond standard security measures and requires you to enter four pieces of information before you can access your information: |
Username |
Password |
Country of Residence |
Unique User ID |
| Only when you use your password and encrypted ID will your health information—from your various health providers, hospital, lab or pharmacy—be connected and gathered in one place. Only when you decide to compile your LifeOnKey™ file does the data merge into your own personal computer’s browser. |
| Market-Proven Innovation |
| The sophisticated technology of LifeOnKey™ “sits” on top of existing health information management systems and data uploaded from peripheral medical devices. |
| The system provides easy-to-use patient rights-control modules, management tools, tracking of follow-up procedures, and prevention strategies. Most importantly, LifeOnKey™ ensures the privacy of your personal health information. |
| The business intelligence embedded in LifeOnKey™ will also allow you to view your health status privately and securely, against a larger LifeOnKey™ population. Imagine being able to compare your treatment or outcomes against a real-time, documented view of people with similar ailments or diagnoses—all real health data aggregated, never individual information. |
| The Intersection of Technical Security and Consumer Confidence |
| LifeOnKey's secure network is managed on a continuous on-demand basis, empowering you to track and manage your medical condition or health goals. This tracking is based on your auto-populated personal health record and any health information you have uploaded to the system from external medical devices. This allows for integrated management of special health conditions, and relevant health markers such as stress cortisol, blood pressure, cholesterol count, and blood sugar levels. LifeOnKey™ members are able to upload confidential and potentially lifesaving information and also track lifestyle behaviors—like diet and exercise—to promote self-care and preventive health. |
| Meeting the Needs of Consumers |
| LifeOnKey™ was specifically designed to answer consumer and patient needs, which have been recognized by leading health care leaders such as the Agency for Healthcare Research & Quality, The Robert Wood Johnson Foundation and The Markle Foundation: |
| 1. |
Individuals should be able to access their health and medical data conveniently and affordably |
| 2. |
Individuals should be able to authorize when and with whom their health data are shared |
| 3. |
Individuals should be able to designate someone else, such as a loved one, to have access to and exercise control over how their records are shared |
| 4. |
Individuals should receive easily understood information about the ways their healthcare data may be shared |
| 5. |
Individuals should be able to review which entities have had access to their personal health data |
| 6. |
Electronic health data exchanges must protect the integrity, security, privacy, and confidentiality of an individual’s information. |
With over 20 Centers of Excellence worldwide, LifeOnKey™ provides leading-edge technological advances combined with ‘real-world’ experience—all in an unrelenting effort to provide you with a secure, consumer-centric personal health record platform. |
